Medical Device Authorization

Medical Device Authorization Notice: Sale of certain products by H&H Associates, Inc. is restricted to Federal and Military organizations or to distributors providing this product to Federal and Military organizations.

Sale of these certain products to all other parties requires a Medical Device Authorization. This authorization is required to ensure that only those persons who are trained and are authorized to use the product sold by H&H. The products that are subject to this prior authorization are:

• H&H Emergency Cricothyrotomy Kit
• H&H Tension Pneumothorax Needle
• H&H Chest Wound Kit
• H&H Chest Tube Insertion Kit
• H&H Damage Control Surgery Wound Dressing Kit
• H&H Lateral Canthotomy Kit

Prior to any sale of any product or any kit to non-Military organizations, H&H may require completion of a Medical Device Authorization as a contingency of this order. Distributors may use the our form to secure authorization prior to sale.
